*Minimum employee participation at an onsite is 20. If attendance falls below 20 on the day of the event, a fee of $350 will be applied.

Cancelation Policy: Due to staffing and planning, we require 7 days prior cancellation notice for any on-site flu clinics. Any cancellations with less than a 7-day notice will incur a $100 rescheduling fee.
